Who We Are

Mission Statement

The Affinity Center is dedicated to being the finest and most comprehensive facility of its kind. We are passionately devoted to providing quality care to an under-served and often misunderstood group of people. The real power to change is in the hands of our clients and their families. Understanding and cultivating that power, the power to change one's life and to realize one's full potential, is our primary goal.

History

Tom D'Erminio, LISW and Doug Pentz, Ph.D founded The Affinity Center on the belief that the consumer is the most important person in the treatment process and that listening to them is our first priority.

The Affinity Center opened its doors in 1996 in response to a need for integrated, comprehensive services for people with ADD/ADHD in our community. The identification of ADD/ADHD as a "real" affliction that can significantly harm adults, children and their families, was a relatively new phenomenon. Thus, people seeking help for this disorder were, often lost within a complicated maze of healthcare professionals who have not yet had many opportunities to study, identify and appropriately treat this disorder.

The Affinity Center Concept was developed over a two year period. An extensive survey of consumers and health care providers led to our multidisciplinary approach. As a result of that feedback, we have made significant strides as we look back over the last 10 years.

Today

Our clients come to us with a variety of concerns and other psychiatric conditions, not just ADD; therefore, we offer counseling services, coaching, medical consultation, educational evaluation and recommendations, for ages preschool through late adulthood. Futhermore, we understand, now, more than ever, that attention is the result of a complex neuro-biological system in which the optimal treatment results are achieved within the Affinity Center model.

We have also come to appreciate that the "complex client", that person or family with varied and specialized needs, still is not able to find adequate care in the community. The Affinity Center specializes in treating this type of client and family. Our staff physician has a broad interest and
expertise in the treatment of a variety of medical and emotional illnesses, so we are able to
accommodate a breadth of client needs. We also have competent, educators, pyschologists,
clinical social workers, and speech and language specialists.

We are committed to remain flexible and open to options that may enhance the lives of our clients. This flexibilty is one of the key qualities that clients tell us set us apart from other treament providers in the community.

Staff Profiles

Misti Burress joined the Affinity Center in April 2007 as an addition to the Support Staff. Originally from Lexinton KY, Misti attended the University of Kentucky for two years studying business management. Misti has one year of medical office experience as a receptionist and three years of experience as a phlebotomist.

Elizabeth Burroughs, MA is the educational consultant of The Affinity Center. She works as a liaison between students and their teachers and schools to meet the students’ individual learning needs. She also enjoys helping students and adults examine their organizational and time management skills to foster more success and self-sufficiency in their lives. She worked as a speechwriter, public relations and marketing professional for 12 years in Fortune 500 corporations in New York City. Her “second career” in teaching began over a decade ago, giving her teaching experience with junior high, high school and college students in New Jersey and within Cincinnati.

Amanda Conrad, M.A. CCC-SLP is a speech/language pathologist who has provided consultation services to the Affinity Center for the past 5 years. She provides traditional speech and language services to children and adults which include assessments and treatment of oral expression, language processing, and written language skills. In addition, she provides social/pragmatic language skill assessment and treatment both individually and in groups. She has work experience in the hospital setting, private clinic, and public schools over the past 25 years.

Tom D'Erminio, LISW is co-founder and co-director of The Affinity Center. He has been a psychotherapist working with children, adults, couples and families for over 30 years. As an adult with ADD/ADHD, his participation in the development of a center for the treatment of ADD/ADHD was the result of a personal and long awaited dream come true.

Richard Kloss, M.S.W., LISW has provided psychotherapy and consultation services to individuals and couples since 1979. In addition to being in private practice since 1982, he was the director of an outpatient center within the mental health system from 1984-1992. During his years in community mental health, he also held a position as Adjunct Faculty, University of Louisville. His interest in ADD/ADHD led him to join the Affinity Center in 2002. Other clinical interests include treatment to depression and anxiety in adults and adolescents.

Andrea Johnson, Psy.D. is our most recent addition to the staff of The Affinity Center. Andrea is a recent graduate of Xavier University, and has already proven herself to be an excellent therapist. As a psychologist at Affinity, Dr. Johnson does psychological testing and therapy with both children and adults.

Doug Logan, MD joined the Center in January 2007. Dr. Logan began his career as a junior high school teacher of history and math before deciding to return to school for his medical degree. He practiced primary care internal medicine and also taught medical students for 27 years before retiring from practice in 2006 to take a position with a local firm specializing in clinical pharmaceutical research in novel drugs for cardiovascular diseases and diabetes. Working at The Affinity Center will allow him to continue to participate a few hours per week in his “first love,” which is patient care, and in an area of medicine in which he had developed an increasing interest over the most recent several years of his own practice.

Chris Mayhall, PhD is a psychologist with specialized training in child development, psychological assessment, individual and family therapy at Xavier University, Ohio State University, and in Houston. She worked in Cincinnati for 5 years at The University Psychiatric Services and was Director of The Child and Family Division there. She joined The Affinity Center staff in 1998. Her wide array of clinical interests and duties include psychological assessment, AD/HD diagnosis and treatment, working with teachers and schools to meet children's needs, and the treatment of anxiety and depression in people of all ages.

Susan Montauk, MD is the Medical Director of The Affinity Center and a professor of Family Medicine at the University of Cincinnati College of Medicine. In addition to being a family physician and an ADD/ADHD specialist, she is a nationally recognized educator. Medical students and residents throughout the country use her book, Guide to the Family Medicine Clerkship, during their training. In addition, she enjoys a full family life with her husband, two sons, two cats, one dog, and a large flower garden.

Doug Pentz, PhD is co-founder and co-director of The Affinity Center. He is a clinical psychologist and has been a psychotherapist for thirty years. The early part of his career was spent working in a wide variety of mental health settings, including fourteen years of family therapy in the University of Cincinnati Medical Center and several years serving as co-director of the University Psychiatric Services Child and Family Division. He has spent the last decade personally and professionally committed to understanding and treating Attention Deficit Hyperactivity Disorder.

Emily Powell is the Coordinator for Support Services.   She joined The Affinity Center in March 2005.  If you have a business related question or call for information about the Center, you will probably speak with Emily or one of her staff members.

Cathy Siemer joined the Affinity Center in November, 2006 as an addition to the support staff. A Cincinnati native, Cathy studied Business at the University of Cincinnati. She worked in Student Affairs at the University of Cincinnati for 17 years. She managed a dental practice in Hyde Park for 4 years while attending Real Estate School and went on to become licensed as a Realtor.

Henry "Bo" Waite, MD is a longtime friend of Affinity, being one of our first physicians along with Susan Montauk, MD. Bo's return to Affinity offered us the opportunity to assist with those individuals who needed the expertise of a psychiatrist and who previously would have had to go elsewhere for their consultation to have it be a part of their overall treatment here at Affinity. Dr. Waite is board certified in both child and adult psychiatry.
