Access Your Telehealth Appointment Link
HOW TO VIDEO CONFERENCE WITH YOUR AFFINITY CENTER CLINICIAN:
Click on your clinician’s video link a few minutes before your scheduled appointment time to enter the virtual waiting room.
You will be asked to type in your name so that your clinician can see who is signed into the waiting room.
Once in the virtual waiting room, your clinician will begin the appointment by starting the video conference around your scheduled appointment time.
Your clinician will discuss the use of tele-behavioral health services including the potential risks, benefits, and any other necessary information so that you can make an informed decision about your consent for telehealth services. You will be asked to sign a telehealth consent form to acknowledge your consent. Your clinician will explain how to complete and sign this consent form.
Please Note: If your clinician does not enter your doxy visit by 10 minutes after your scheduled start time, please call the office right away.

The Affinity Center prioritizes in-person visits for all clients to ensure optimal treatment. Additionally, our medical providers must follow strict regulations as to the frequency of visits, as well as the type of visit (in-person vs. virtual). Failure to be present for all required medical appointments may result in additional required office visits, delays in receiving medications, and, in rare situations, discontinuation of services.
IF YOU CHOOSE TO ATTEND THERAPY VIRTUALLY, HERE IS SOME IMPORTANT INFORMATION TO CONSIDER:
Clients should receive our courtesy reminder messages with details stating whether or not their appointment is in person. Please be sure that your anticipated virtual appointment is, indeed, scheduled as virtual. Failure to appear for an in person appointment (even if you are logged into your clinician’s Doxy account) can be charged as a missed appointment.
Virtual visits are NOT appropriate as a last minute alternative to a client forgetting an appointment. Same day requests to change an in-person appointment to virtual (with a few exceptions) will be denied, and client will be charged with late cancel fee.
Video conferencing sessions are conducted using Doxy.me, which is HIPAA compliant, secure, and free of charge to our clients.

Access Your Fullscript Portal
Through Fullscript, we are now able to provide all of our clients access to a complete range of both prescription strength and “over the counter” supplements provided by over 300 trusted and industry leading companies, and with a 10% discount!
